乳腺EIS检测评价年轻女性罹患乳腺癌危险度的前瞻性研究 被引量:4

Electrical impedance scanning in estimating risk for development of breast cancer in young women:a prospective study

摘要 目的评价乳腺电阻抗扫描成像(EIS)对年轻女性乳腺癌病人的敏感度、特异度,计算其相对危险度,决定是否采用更精确的影像学检查,以期指导年轻女性乳腺癌病人的早期检测。方法2005年6月至2007年6月6家医疗机构采用前瞻性、多中心的方法对年龄≤45岁的女性进行临床观察研究。对583例准备进行乳腺组织活检的年轻女性病人进行乳腺EIS检测,与其病理结果相对照,计算出其敏感度,特异度,准确度,阳性预测值,阴性预测值,同时计算乳腺EIS检测阳性病人患乳腺癌的发生率。结果583例中143例确诊为乳腺癌。EIS检测的敏感度、特异度、准确度、阳性预测值、阴性预测值分别为86.7%、72.9%、76.3%、51.2%和94.1%。EIS检测结果阳性的年轻女性患乳腺癌的相对危险度为8.67。结论乳腺EIS检测可能成为年轻女性早期筛查乳腺癌的重要方法。 Objective To evaluate the sensitivity and specificity of electrical impedance scanning (EIS) in identifying breast cancer for young women, to caleulate relative risk, and to determine whether the more accurate imaging method should be used in early breast cancer detection for young women based on cost-effective ratio. Methods A prospective clinical study was done in young women aged 45 years and under from 6 hospitals between June 2005 and June 2007. The young women (583 cases) scheduled for mammary biopsy underwent EIS. EIS results were compared with final histopathology results. Study end points included sensitivity and specificity of EIS as well as relative probability of breast cancer of positive patients detected by EIS. Results Of 583 cases, 143 cases were diagnosed as breast cancer. The sensitivity specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of EIS were 86. 7 % ,72. 9%, 76. 3% ,51.2% and 94. 1% respectively. The relative possibility of breast cancer in positive young women detected by EIS were 8.67. Conclusion EIS is likely to become a main approach to screen early breast cancer in young women.
