关节镜辅助手术治疗股骨髁间骨折21例临床分析

Analysis of the clinical from arthroscope-assisted surgical treatment for 21 cases with fracture of femoral intercondyles

目的评价应用关节镜辅助复位技术结合动力髁钢板(DCS)固定股骨髁间骨折的临床效果。方法采用关节镜辅助复位技术、最少的软组织剥离以局部显露、DCS固定技术治疗股骨髁间骨折21例。结果随访时间11个月～3年1个月,平均为1.5年,骨折平均愈合时间为2.3个月。应用王亦璁的功能评定方法评价膝关节功能,优9例,良11例,可1例。结论关节镜辅助复位技术结合DCS固定技术是股骨髁间骨折的一个较好的选择。

Objective According to analysis of the clinical outcomes from arthroscope-assisted treatment with fracture of femoral inter-condyles, to evaluate the role in the application of arthroscope-assisted technique combined with DCS fixation. Methods Arthroscope-assisted technique combined with DCS fixation was performed with intra-condyles of femur, which has the slightest soft tissue injury. Results During 1.5-year follow-up, the mean time for the union of fracture was 2.3 months, and the weight loading within 3.8 months. All cases with fracture achieved healing with neither dysmorphosis nor infection, and then flexion degree of knee joint averaged 121.23°±10.50°. The results evaluated from the modified scoring system of knee joint respectively were： excellent 9 case,or fine 11 case, or normal 1 case, and none of the abnormal. Conclusion Arthroscope-assisted technique combined with condylar dynamia nickelclad fixation is an ideal choice for fracture of femoral intercondyles, with the slightest soft tissue injury.
