青光眼小梁切除术后浅前房的相关因素分析及防治

The Reasons Analysis and Treatment of Shallow Anterior Chamber after Trabeculectomy

目的分析青光眼小梁切除术后浅前房的发生原因、治疗及预防措施。方法对不同类型青光眼146例(177眼)小梁切除术后浅前房45例(79眼)进行原因分析及治疗效果观察。结果本组177眼中术后发生浅前房79眼,占44.68%。其原因滤过过强55眼,结膜瓣漏水5眼,脉络膜脱离9眼,虹膜睫状体炎7眼,睫状环阻滞性青光眼3眼。其中57眼保守治疗、22眼手术治疗恢复前房。结论浅前房是青光眼术后早期主要并发症,滤过过强是主要原因,应术前控制眼压,术后密切观察,针对不同病因做出相应治疗以及时恢复前房。

Objective To analyze the reasons,treatment and preventive measure of shallow anterior chamber after trabeculectomy for glaucoma. Methods A total of 146 eases（ 177 eyes） with different type glaucoma underwent trabeculectomy, the shallow anterior chamber 45 cases（79 eyes） were observed and treated. Results Shallow anterior chamber occurred in 79 eyes（44.68%）. The reason included excessive smooth of drainage 55 eyes,conjuncfival flap water leaking 5 eyes, detachment of choroids 9 eyes, iridocyclitis 7 eyes, obstructive glaucoma in annulus ciliaris 3 eyes, among of total 57 eyes adapted expectant treatment, 22 eyes were recovered anterior chamber by operation. Conclusion Shallow anterior chamber was early main complication underwent trabeculectomy. Strong filter was the primary cause,should control ophthalmotonus before operation and observed closely after operation,make the corresponding measure in view of the different cause to restored anterior chamber promptly.
