肝细胞腺瘤的螺旋CT诊断

The Diagnosis of Hepatocelllular Adenoma with Spiral CT

目的探讨肝细胞腺瘤的螺旋CT表现,提高CT正确诊断水平。方法回顾性分析经手术病理证实的7例肝细胞腺瘤的螺旋CT资料,并与手术病理所见对照。结果7例肝细胞腺瘤均为单发肿块,无肝硬化背景,直径3.6～7.4cm;螺旋CT平扫:7例肝细胞腺瘤均呈边界清楚、稍低密度占位灶,除因出血、坏死外密度较均匀;注射碘海醇后,病灶动脉期明显强化,门脉期和延迟期呈等或稍低密度。结论肝细胞腺瘤螺旋CT表现有一定特点,利用CT平扫+多期动态增强检查,再密切结合临床资料有助于肝细胞腺瘤的明确诊断。

Objective To explore the spiral CT appearances of hepatocelllular adenoma and improve CT diagnostic accuracy. Methods The CT data of 7 cases hepatocelllular adenoma proved by operation and pathology were retrospectively analyzed and comparatively with pathologic findings. Results All cases were single mass and without liver cirrhosis. The diameter of lesiongs ranged from 3.6 -7.4em;In plain CT,the lesions were slight hypodensity with margins clear except hemorrhage or necrosis. After injected into lopamiro,the hepalocelllular adenomas shown marked enhaneemenl in the arlerial phase,and in the portal venous and delay phase,the lesions shown isodensity or slight hypodensity. Conclusion Hepatocelllular adenoma CT signs have certain characteristics,make use of spiral CT plain scan and multi-phase exam,close links clinical data has important value in diagnosing the hepatocelllular adenomas.
