第三代头孢耐药大肠埃希菌表型及基因型研究 被引量:2

摘要 目的检测3代头孢耐药的大肠埃希菌的表型及基因型,了解产ESBLs大肠埃希菌的最佳检测底物及耐药机制,以利于耐药性的控制与指导临床用药。方法收集各种标本对3代头孢耐药的大肠埃希菌242株,用双纸片确诊法检测ESBLs;标准纸片扩散法(Kirby-Bauertest)检测耐药性;聚合酶链反应(PCR)对ESBLs阳性菌进行耐药基因检测并测序。结果检测ESBLs的最佳底物是头孢噻肟。242株大肠埃希菌中,产ESBLs146株,占60.3,产酶株对头孢噻肟等16种抗生素的耐药率比不产酶株都有不同程度增加,但亚胺培南对所有产酶株都敏感。146株产ESBLs细菌中,120株产CTX-M型β-内酰胺酶,52株产TEM型β-内酰胺酶,12株产OXA型β-内酰胺酶,除66株为CTX-M单独存在外,其余均与TEM、OXA共存。结论临床及药敏资料证明产ESBLs大肠埃希菌是重要的医院内感染细菌,且呈多重耐药,治疗中碳青霉烯类药物是最可靠的;含β-内酰胺酶抑制剂的复合制剂耐药程度也较高,应适当增加剂量,并可与阿米卡星联合应用。表型为产ESBLs的菌株中,主要基因型为CTX-M型β-内酰胺酶,其次为TEM型。
