小剂量低分子肝素钙对过敏性紫癜患儿的肾保护作用及其机制探讨

摘要 目的:探讨小剂量低分子肝素钙对过敏性紫癜(HSP)患儿的肾保护作用及其机制。方法:应用酶联免疫吸附法检测和比较低分子肝素钙治疗组和对照治疗组患儿治疗前后血浆血栓调节蛋白(TM)水平,并随访6个月,另取30例正常儿童作为正常对照组。结果:治疗前低分子肝素钙组与对照治疗组血栓调节蛋白无差异;治疗后低分子肝素组血栓调节蛋白较对照组明显降低。随访6个月后,治疗组紫癜性肾炎发生率较对照组明显降低。结论:低分子肝素钙可能通过减轻内皮细胞损伤降低过敏性紫癜肾受累的发生率。 Objective: To explore protective effect of low weight molecular heparin calcium on kidneys of children with Henoch- Schonlein Purpura (HSP) and its mechanism. Methods: 95 cases with HSP were randomly divided into heparin calcium treatment group and control treatment group. 30 cases healthy children were enrolled as normal control group. Plasma levels of thrombomodulin(TM) were detected by enzyme - linked immunosorbent assay (ELISA) respectively. These children were followed up for over six months. Results:No significant differences in levels of TM were found between the treatment group and control group, but the levels of Tmin the treatment group were obviously lower than those in control group. During the follow - up course, the morbidity of purpura nephritis in the treatment group was lower than that of control group. Conclusions: The plasma levels of TM decrease after low - weight heparin calcium treatment, which can decrease the morbidity of renal injury in children with HSP treatment.
