冠心病患者糖基化白蛋白与肿瘤坏死因子-α水平的关系 被引量:3

Relationship between Serum Level of Glycated Albumin and Tumour Necrosis Factor-α in Coronary Heart Disease

摘要 目的:探讨冠心病患者血清糖基化白蛋白(GA)与肿瘤坏死因子-α(TNF-α)水平之间的关系。方法:用病例对照研究方法,以冠脉造影确诊的冠心病患者200例为冠心病组,以200例无糖尿病及冠心病人群为对照组,测定各组血清白蛋白(Albumin)、GA及TNF-α水平,分析上述各指标之间及与冠心病的关系。结果:冠心病组血清GA15.74%(95%CI15.43~16.01);TNF-α79.23±7.63ng/L(95%CI68.17~91.26)分别高于对照组GA14.71%(95%CI14.51~14.92,P<0.01)和TNF-α64.05±5.81ng/L(95%CI55.31~72.04,P<0.05)。血清GA与TNF-α水平呈正相关(r=0.346;校正年龄和血糖后,偏相关系数r=0.339,P<0.01),多元逐步回归分析提示,TNF-α水平对血清GA水平影响最显著(β=0.334,P<0.01),GA是唯一对TNF-α水平有显著影响的变量(β=0.347,P<0.01)。结论:冠心病患者血清GA水平显著升高,且与TNF-α水平正相关,可能与冠心病患者的炎症状态有关。 Objective:To investigate the relationship between serum level of glycated albumin(GA)and tumour necrosis factor-α(TNF-α)in coronary heart disease.Method:This case-control study included 200 patients with coronay heart disease(CHD group)and 200 controls.Serum levels of albumin,glycated albumin and TNF-α were measured.The relationship between GA and TNF-α,and its association with coronary heart disease were analyzed.Results:Serum level of GA(15.74%,95% CI 15.43~16.01),and TNF-α\[(79.23±7.63)ng/L,95% CI 68.17~91.26\] were significantly higher in CHD group than in control group GA(14.71%,95% CI 14.51~14.92,P<0.01),TNF-α\[(64.05±5.81)ng/L,95% CI 55.31~72.04,P<0.05\].Serum GA level correlated significantly with TNF-α(pearson correlation coefficients r=0.346,partial correlation coefficients r=0.339,P<0.01).Multiple stepwise regression analysis revealed that serum TNF-α level was the most significant factor for determines of serum GA level(β= 0.334,P<0.01);the serum GA level(β= 0.347,P<0.01)was the only significant factor in determining of TNF-α.Conclusion:Seurm level of GA was elevated in coronary heart disease and correlated positively with seurm level of TNF-α,this may be related to inflammary process in coronary atherosclerosis.
