不同大小超高分子聚乙烯颗粒对兔膝关节周围组织巨噬细胞及炎症因子表达的影响 被引量:1

The effect of worn particles of ultrahigh molecular weight polyethylene on the macrophages and their expression of inflammatory cytokine of peripheral tissues in rabbit knee joints

摘要 目的探讨不同大小超高分子聚乙烯(UHMWPE)颗粒对兔膝关节周围组织巨噬细胞及炎症因子表达的影响。方法实验组:将3种大小(平均直径)为1.33、5.92、58.87μm的UHMWPE颗粒用无菌磷酸盐缓冲液(PBS)配成0.1 g/L混悬液注入日本大耳白兔右侧膝关节腔。对照组:不植入UHMWPE颗粒,其余条件均与实验组相同。分别于第8、16、24周取膝关节周围组织,进行病理组织学检查,ELISA法检测标本组织中白细胞介素(IL)-1β、IL-6、肿瘤坏死因子(TNF)-α的变化。结果与对照组相比,1.33μm组、5.92μm组可见吞噬UH-MWPE颗粒的巨噬细胞,58.87μm组可见大量巨噬细胞与多核巨细胞包绕UHMWPE颗粒。实验组IL-1β、IL-6、TNF-α的表达与对照组比较均升高(P<0.05),而1.33μm、5.92μm 2组IL-1β、IL-6、TNF-α的表达高于58.87μm组(P<0.05)。结论不同大小的UHMWPE颗粒都会激活巨噬细胞并刺激其释放炎症因子,在其他条件相同的情况下,小颗粒的刺激作用比大颗粒更强。 Objective To investigate the effects of UHMWPE particles of varied sizes on macrophages and their inflammatory cytokine expression in peripheral tissues of rabbit knee joints. Methods. 60 Japanese white rabbits were divided into 4 groups at random and were injected with UHMWPE particles of three diameters ( 1.33 μm, 5.92μm and 58.87 μm, respectively) at 0.1 g/L concentration into the knee joint capsules. At the 8th week, 16th week and 24th week after particle injection, assessment of histological examination and determination of the three cytokines - - IL - 113, IL- 6 and TNF-α were performed respectively. Results Optical microscopy indicated that there were a lot of UHMWPE particles in peripheral tissue of the capsule. 1.33 μm and 5.92 μm foreign - body particles were phagocytosed by the neighboring macrophages and 58.87 μm particles were surrounded by muhinucleated giant cells, fibroblasts, etc. All the rabbits injected with UHMWPE particles had enhanced the cytokine expression of IL - 1β, 1L - 6 and TNF - α re- gardless of particle sizes when compared with control animals injected with sterile phosphate buffered saline ( P 〈 0.05 ). UHMWPE particles of 1.33 μm and 5.92 μm in size provoked macrophages to release IL - 1 β, IL - 6 and TNF - α higher than 58.87 μm particles (P 〈 0.05). Conclusion UHMWPE particles of each size are capable of activating macrophages and inducing inflammatory cytokine release, while small particles (1.33 μm and 5.92μm) seem to provoke a stronger inflammatory response than larger particles (58.87 μm) under the conditions with equal particle volumes.
