术后早期炎性肠梗阻68例诊治体会

Experiences in diagnosis and treatment of early postoperative inflammatory small intestinal obstruction:a report of 68 cases

目的探讨术后早期炎性肠梗阻的临床特点及处理方法。方法分析68例术后早期炎性肠梗阻的临床特点及治疗结果。结果68例均经胃肠减压,应用糖皮质激素和生长抑素,肠外营养等支持疗法,平均治愈时间为9.5d,无一例再手术。结论术后早期炎性肠梗阻多发生在术后5～8d,可表现为典型的肠梗阻体征,多由小肠无菌性炎症广泛肠粘连引起,多数病例采用保守疗法可治愈。

Objective To investigate the principles of diagnosis and treatment of early postoperative inflammatory small intestinal obstruction （EPISBO）. Methods Sixty-eight cases with early postoperative inflammatory small bowel obstruction were analyzed retrospectively. Results sixty - eight cases were all cured in the mean period 9.5 days through gastroenterology decompression, glucocorticoid and sematostatin. TPN and other medications. There was on reoperations. Conclusion EPISBO mostly occurs in about 5 - 8days postoperatively. EPISBO has the clinical features. Most cases were caused by extensive adhesion resulted from aseptic inflammations. Conservative treatment should be the first choice for EPISBO patients.
