腓肠神经营养血管肌皮瓣治疗小腿下段及足踝部组织感染缺损 被引量:6

摘要 目的探讨腓肠神经营养血管肌皮瓣逆行转位修复小腿下段及足踝部软组织缺损的可行性。方法对12例小腿中下段及足踝部软组织缺损或骨缺损、骨髓炎的病例,采用逆行腓肠神经营养血管肌皮瓣转移方法进行治疗。结果12例中11例完全存活,1例皮瓣远端皮肤部分坏死,经换药瘢痕愈合。随访5—54个月,平均21个月,皮瓣质地优良,外观满意,感染治愈均无复发。结论腓肠神经营养血管肌皮瓣解剖简单、血供可靠,抗感染能力强,是临床上治疗小腿及足踝部软组织缺损并感染的良好选择。 Objective To evaluate the feasibility of neurovascular sural flap in treatment of osteomyelitis or infection at the lower leg, ankle and foot with soft tissue defects and bone defects. Methods Twelve cases of soft - tissue defects and osteomyelitis of the lower leg and foot associated with or without bones defects were treated with neurovascular sural flap. Results Of all the 12 flaps, 11 flaps survived at the first intention. Partial necrosis appeared in 1 flap but was cured at the second intention. The follow-ups of 5 to 54 months( mean 21 months) showed that the appearance and function were satisfactory in all cases. Conclusion To repair soft tissue defect at the lower shank, foot and ankle with osteomyelitis,the neruovascular sural flap is a satisfactory method.
