COPD患者合并医院感染50例临床分析

Clinical analysis of 50 patients of COPD with infection in the hospital

目的探讨COPD患者合并医院感染的临床特点及防治措施。方法对50例COPD患者医院感染情况进行回顾性分析。结果医院感染率为20.3%,病死率为26%,明显高于COPD患者非医院感染的病死率(P<0.01)。感染部位以下呼吸道为主(78%),其次为上呼吸道、泌尿道、胃肠道等。感染发生率与住院时间成正比。感染与长期连用多种抗生素有关。结论COPD患者医院感染发生率及病死率较高,在积极治疗基础疾病的同时,应合理使用抗生素,缩短住院时间,减少医院感染的发生。

Objective To analyze the clinical characteristics on the COPD patient with infection in the hospital,inquire into its prevention and treatment measures. Methods This text chose 246 hospitalized patients of COPD from April 2005 to April 2008 in our hospital,and among them 50 patients had infection in the hospital. The patients were measured on：the occurrence rate of infection,the death rate, the place of infection, the causes of infection, the time in the hospital, use of antibiotic, etc, carried on review analysis. The data was analyzed using x±s and X^2 test. Results For the patients of COPD, the rate of infection in the hospital was 20. 3 %, the death rate was 26% with infection in the hospital. The rate of the patient of COPD with infection in the hospital died of illness was higher obviously than the patient of COPD without infection in the hospital （ X^2 = 17.24, P 〈 0. 01 ）. 78 % of the patients had respiratory tract in the main part, followed by the upper respiratory tract, urethra, the stomach and intestines tract etc. The occurrence rate of infection was directly related to the hospital-stay time. The longer the hospital stay, the higher the injection rate （ X^2 = 20. 83 ,P 〈 0. 01 ）. Infection was also related to the use of various antibiotics. Conclusion The rate of hospital infection and the death rate are higher in COPD patient. For the patient of COPD receiving treatment for the basic illness,they should be given nutrition support,and the body＇s immunity enhancement. Their hospital stay time should be shortened and their use of antibiotic cautious, to prevent and reduce the occurrence of the hospital infection.
