与腹股沟疝手术有关的应用解剖

Applied Anatomy Related to Operation for Inguinal Hernia

目的为腹股沟疝手术提供应用解剖。方法解剖８３侧成人腹壁标本，观测髂腹下神经和髂腹股沟神经行径的规律性；解剖１２侧成人腹部标本，观察生殖股神经生殖支的局部关系。结果（１）髂腹下神经、髂腹股沟神经、两神经共干与腹股沟韧带中点垂直距离分别为２２．４±７．７ｍｍ，１０．５±４．４ｍｍ，２０．６±４．２ｍｍ；（２）生殖股神经的生殖支位于腹环外侧６．５±４．０ｍｍ，占６２．５％；生殖股神经的生殖支位于精索（或子宫圆韧带）背侧占６６．７％。结论提出在术中保护神经方法，以减少并发症的发生。

Objective\ To provide applied anatomy for the operation on inguinal hernia.\ Methods\ The distribution of the iliohypogastric and ilioinguinal nerves was studied on 83(sides) adult abdominal wall specimens;local relation of genital branch was observed on 12(sides) adult abdominal specimens.\ Results\ (1)The vertical distances between the middle point of inguinal ligament and iliohypogastric nerves.ilioinguinal nerves.common trunk of two nerves were 22.4±7.7 mm,10.5±4.4 mm,20.6±4.2 mm respectively.\ (2)Ramus genitalis was located in the external side of deep inguinal ring(the average distance is 6.5±4.0 mm),accounting for 62.5%. Ramus genitalis was located in the dorsal side of spermatic cord or round ligament of uterus,accounting for 66.7%. Conclusion\ A method that may protect nerves and reduce complications during operation is afforded.
