网织血小板检测在血小板减少症中的临床价值

Clinic value of immature platelet fraction in thrombocytopenia

目的:探讨检测网织血小板对血小板减少症的诊断价值。方法:随机选取原发性血小板减少性紫癜31例,弥散性血管内凝血19例,再生障碍性贫血13例,急性白血病初发/复发42例,健康对照组93例,用SysmexXE-2100型全自动血液分析仪检测网织血小板。结果:血小板骨髓生成活跃组(原发性血小板减少性紫癜、弥散性血管内凝血)网织血小板百分比显著高于健康对照组(P<0.01);血小板骨髓生成抑制组(再生障碍性贫血、急性白血病)网织血小板百分比和健康对照组差异无统计学意义;区分血小板骨髓生成活性方面,网织血小板检测明显优于平均血小板体积检测。结论:网织血小板可反映血小板减少症中血小板骨髓生成活性,对其诊断有一定临床意义。

Objective To determine the clinic value of immature platelet fraction to thrombocytopenia. Methods The study included 31 patients with idiopathic thrombocytopenic purpura, 19 patients with dessiminated intiavascular coagulation, 13 patients with aplastic anemia, 42 patients with acute leukemia and 93 healthy people. Immature platelet fraction was detected with hematological autoanalyser. Results Immature platelet fraction in patients with idiopathic thrombocytopenic purpura and dessiminated intiavascular coagulation was significantly higher （P〈0.01） than that in health control group. Immature platelet fraction in patients with aplastic anemia and acute leukemia was not significantly higher than that in control group. In receiver operating characteristic curve for diagnosis of increased platelet production and Normal/Suppressed platelet production, immature platelet fraction was significantly more useful than mean platelet volume. Conclusion Immature platelet fraction reflects the pathology of thrombocytopenic disorders, and that measurement of immature platelet fraction is useful for the differential diagnosis and analysis of platelet kinetics.
