局麻下Modified Kugel^(TM)疝补片在老年腹股沟疝修补术中的应用

Clinical application of the tension-free herniorrhaphy with Modified Kugel^(TM) patch in the old patients with local anesthesia

目的探讨局麻下Modified KugelTM疝补片在老年腹股沟疝无张力修补术中的应用效果。方法回顾分析2005年4月至2007年5月我科使用巴德公司KugelTM疝补片在局麻下修补老年腹股沟疝87例的临床资料(包括手术时间、手术出血量、术后并发症及住院时间)。结果平均手术时间42 min,术中平均出血量37.5 ml,86例切口甲级愈合,术后下床活动时间平均6.5 h,切口疼痛7例,术后出现皮下血肿1例,阴囊水肿3例,局部有异物感2例。随访时间最长39个月,最短3个月,无一例复发。结论局麻下Modified KugelTM疝补片在老年腹股沟疝的应用中优点明显、安全可靠。

Objective To summarize the clinical effect of the tension-free herniorrhaphy with Modified Kugel^TM patch and local anesthesia in the old patients in our department. Methods From April 2005 to May 2007, 87 old patients suffering from inguinal hernia underwent tension-free herniorrhaphy using Modified Kugel^TM patch with local anesthesia, the clinical data （including operating time, the hemorrhage amount, postoperative eomplications, the recurrence rate and hospital stay. ） were analyzed retrospectively. Results All the patients were operated successfully. The average time of operation is 42 minutes, the average hemorrhage amount in the operation is 37.5 ml and the average walking time after operation is 6.5 hour. Wound healing of 86 patients fall under Class A. the postoperative 7 Incision pain , 1 subcutaneous hematoma ,3 scrotum hydroeeles and 2 patients part bary sehsation feel. The follow-up period ranged from 3 to 39 months and no recurrence was observed. Conclusions The advantages of the tension-free herniorrhaphy with Modified KugelTM patch and local anesthesia on the old patients suffering from inguinal hernia are obvious, and the technique is safe and reliable.
