摘要
目的探讨岩浅大神经与岩骨周围结构之间的位置关系,为经岩骨前部入路中定位岩骨内重要结构及磨除颞骨岩部提供解剖学资料。方法采用10%福尔马林固定的成人头颅湿标本16具(32侧),观察并测量岩尖周围结构之间的位置关系;模拟经岩骨前部入路操作,对手术全程及术野的暴露范围进行观测。结果岩浅大神经为经岩骨前部入路中的一个重要标志,其构成手术入路中磨除岩尖的最外界。结论熟悉颞骨岩部的显微解剖及其周围结构之间的位置关系,能在经岩骨前部入路的手术操作中最大限度地扩大岩斜区的暴露范围,减少并发症的发生。
Objective To explore the location relations between greater superficial petrosal nerve and peripetrous structures,so as to provide anatomic data for locating the important structures in the petrous bone and drilling petrous pyramid during the anterior transpetrosal approach. Methods 16 adult cadaveric head specimens fixed in formalin were subjected to mimic operation of anterior transpetrosal approach. Of the both sides of the specimens, the location relations of petrous and peripetrous structures were observed and measured,and the petroclival exposure area was also detected. Results The greater superficial petrosal nerve was an important landmark and it was the lateral boarder for drilling petrous pyramid in the course of the anterior transpetrosal approach. Conclusion The petroclival exposure could be maximized in operations through anterior transpetrosal approach with fewer complications, providing that the microsurgical anatomy of the petrous bone as well as the location relations between the related landmarks are well familiarized with.
