鼻咽癌组织表达VCAM-1与局部淋巴细胞浸润的关系

摘要 目的:观察鼻咽癌组织表达血管粘附分子-1(VCAM-1)与局部浸润淋巴细胞的关系。方法:收集51例鼻咽癌和22例慢性鼻咽炎石蜡标本和临床资料,观察癌巢间淋巴细胞浸润程度,计数癌巢内淋巴细胞数量;用VCAM-1、CD3、CD4、CD8单克隆抗体进行免疫组化染色,并判断鼻咽癌表达VCAM-1程度,计数CD3+和CD4+、CD8+T细胞。结果:(1)癌细胞表达VCAM-1的鼻咽癌病例达92.16%。(2)鼻咽癌细胞表达VCAM-1与淋巴结转移与否、分化程度和临床分期均无关(P>0.05)。(3)鼻咽癌细胞表达VCAM-1与巢间淋巴细胞浸润程度、巢内淋巴细胞数量、CD4+、CD8+T细胞数量均无关(P>0.05)。(4)鼻咽癌细胞轻度表达VCAM-1时组织中CD3+T淋巴细胞明显增多(P=0.009),但不显示强度依赖性作用。结论:(1)VCAM-1在鼻咽癌细胞表达率很高,其表达强度与转移和临床分期无关。(2)鼻咽癌细胞表达VCAM-1与正常细胞表达VCAM-1的生理作用有所不同,不显示明确地吸引淋巴细胞的作用。 Objective: To observe the relation between expression of vascular cell adhesin molecule-1 (VCAM-1) on nasopharyngeal carcinoma and infiltrating lymphocytes. Methods: The lymphocytes infiltrating in stroma and in carcinoma nest were observed on paraffin slides coming from 51 cases with nasopharyngeal carcinoma (NPC). Meanwhile the expression of VCAM-1 and the number of T lymphocytes and CD^3+ CD^4+ and CD^8+ sub- set of T lymphocytes were counted on immunochemical staining slides. Results: ( 1 ) 92.16% NPC cases (47/ 51 ) expressed the VCAM-1 at different degrees. (2) No relation existed between expression of VCAM-1 and either node metastasis, tumor differentiation and stage, or infiltrating lymphocytes, CD^4+ and CD^8+ T cell subsets. (3) Weak expression of VCAM-1 by nasopharyngeal carcinoma cell correlated with increased number of CD^3+ T cell( P = 0.009), but there was no intensity dependent relation between two events. Conclusion: ( 1 ) VCAM-1 highly expressed on NPC but independently of the metastasis, tumor differentiation and stages. (2) The VCAM- 1expressed by NPC cells did not show definite effect of attracting the lymphocytes as that by normal ceUs did.
