温血停跳液间断灌注心肌保护在心脏瓣膜置换术中的应用 被引量:3

摘要 目的探讨温血停跳液间断灌注心肌保护方法在心脏瓣膜置换手术中的应用效果。方法心脏瓣膜置换术患者64例,用温血停跳液间断灌注组(Ⅰ组)34例,传统冷晶体停跳液间断灌注组(Ⅱ组)30例,观察比较两组的CPB时间、主动脉阻断时间、主动脉开放后心脏自动复跳率、呼吸机辅助时间、术后低心排发生率、术后住院天数和多巴胺使用情况。结果两组病人在年龄、心胸比率、心功能及手术类型方面基本类同。CPB时间、主动脉阻断时间差异无统计学意义(P>0.05)。而Ⅰ组主动脉开放后心脏自动复跳率高,呼吸机辅助时间、术后低心排发生率、术后住院天数、多巴胺用量及使用时间明显低于Ⅱ组,差异有统计学意义(P<0.05)。两组病人手术顺利,痊愈出院。随访3个月至2年,心功能恢复良好。结论温血停跳液间断灌注心肌保护方法临床应用效果好,且温血停跳液间断灌注比冷晶体停跳液间断灌注具有更好的心肌保护效果。
