丙泊酚伍用麻黄碱用于胃镜检查的临床观察

The Clinical Observation of Propofol with Ephedrine Used during Gastroscopy

目的:观察丙泊酚伍用麻黄碱用于无痛胃镜检查对麻醉质量和麻醉安全性的影响。方法:150例ASAⅠ-Ⅱ级患者随机分为三组,每组50例。A组:单纯静脉注射丙泊酚至患者入睡、睫毛反射消失后开始置入胃镜检查;B组:先静脉注射芬太尼0.5μg/kg,2 min后注入丙泊酚至患者入睡、睫毛反射消失后开始置入胃镜检查;C组:先静脉注射芬太尼0.5μg/kg,2 min后注入丙泊酚至患者入睡、睫毛反射消失,再注入麻黄碱0.1 mg/kg后置入胃镜检查。记录各组丙泊酚用量、麻醉效果、血压及心率变化情况、不良反应、苏醒时间及苏醒质量。结果:B组较A组丙泊酚用量显著减少,麻醉效果显著优于A组,但并不能显著减轻丙泊酚对循环的影响。C组较A组丙泊酚用量显著减少,麻醉效果显著提高,且循环稳定,不良反应显著减少,同时较A、B组缩短了苏醒时间并显著提高了苏醒质量。结论:丙泊酚伍用麻黄碱用于无痛胃镜检查可防治麻醉中循环波动、减少不良反应并能缩短清醒时间、改善清醒质量,从面提高麻醉质量和麻醉安全性。

Objective：When we use propofol with ephedrine for gastroscopy,we observed them how to influence the anaesthesia quality and safety.Methods：ASAⅠ-Ⅱ class,150 patients were randomly divided into group A and group B and group C with 50 cases each.Group A：Only inject propofol by intravenous,when the patient fall asleep and the eyelash glint disappeared,gastroscopy was done.Grpoup B：First injected fentanyl citrate 0.5 μg/kg,and two minites later,injected propofol.When he fall asleep and the eyelash glint disappeared,gastroscopy was done.Group C：Same as group B,then injected ephedrine 0.1 mg/kg in and carried on the Gastroscopy.Recorded each group：propofol dosage,the result of the anaesthesia,blood pressure and the heart rate variety,bad response,waking up time and waking quality.Results：Group B used less propofol than group A,the anaesthesia result was better than group A,but it could not reduce the circulatating influence from propofol.GroupC uses Less propofol than group A,but the anaesthesia result was better and the circulation stabile,and less bad response.Group C less waking time was shorta than group A and group B and it raised the waking quality.Conclusion：Propofol with ephedrine were used to do gastroscopy.It could maintain circulation stabled in anaesthesia,reduced the bad response and could shorten the waking time,and improved the waking quality,so it could enhance the anaesthesia quality and safety.
