摘要
目的:探讨老年脑梗死、高血压及冠心病患者血清白蛋白浓度(Alb)、血小板体积(MPV)及红细胞分布宽度(RDW)的关系。方法:选取老年脑梗死(单纯脑梗组51例),脑梗死合并高血压及或冠心病(脑梗高冠组70例),仅有高血压及/或冠心病(单纯高冠组60例),检测各组Alb、MPV及RDW等指标,与对照组(63例)比较。结果:脑梗高冠组Alb水平最低,与单纯高冠组、对照组比较;单纯脑梗组与单纯高冠组、对照组比较,差异均具有非常显著性意义(P<0.01)。脑梗高冠组与单纯高冠组中,Alb与RBC正相关,与RDW负相关。自对照组至脑梗高冠组MPV渐增大,脑梗高冠组与对照组比较,差异具有显著性意义(P<0.05)。脑梗高冠组RDW与单纯高冠组比较,单纯脑梗组与单纯高冠组比较,差异具有显著性意义(P<0.05)。单纯脑梗组RDW与PDW正相关。结论:老年人随着心脑及二者合并病变出现,Alb渐降低,MPV渐增大,RDW增宽;老年脑梗死患者较单纯心血管病变患者更易合并营养不良及隐性缺铁,且MPV的改变与脑血管损伤范围及程度关系密切。
Objective:Measured the relationship between serum albumin,MPV and RDW in elderly ischemic cerebral infarction,hypertention and/or coronary heart disease(CHD).Methods:From July 2003 to March 2006,Serum albumin,RBC,MCV,RDW,PLT,MPV and PDW were in 51 elderly ischemic cerebral infarction only,70 who have ischemic cerebral infarction,hypertension and/or CHD and 60 with hypertension and/or CHD only.All were compared with control group(63 cases)respectively.Results:The serum albumin was significantly lower in ischemic cerebral infarction patients with hypertension and/or CHD than that in hypertension and/or CHD only(P〈0.01),control group(P〈0.01)respectively,also was lower in patients with ischemic cerebral infarction,compared with only hypertension and/or CHD group.In patients who had ischemic cerebral infarction with hypertension and/or CHD and hypertension and/or CHD group only,serum albumin was positive related to RBC,negative related to RDW.MPV was larger in group which had ischemic cerebral infarction with hypertension and/or CHD than that of control(P〈0.05).RDW was higher in ischemic cerebral infarction group than that of patients with hypertension and/or CHD.Conclusion:Along with the appearing of the conditions with cerebral infarction or cardiovascular disease only and coexist of ischemic stroke and cardiovascular diseases,the level of serum Alb was reducing,MPV changed to be larger and RDW was higher gradually.It is suggested that there were malnutrition and lack of iron in the elderly ischemic cerebral infarction patients and the change of MPV was significant related to the severity ischemic cerebral infarction.
