非手术区血肿致开颅术中急性脑膨出的原因分析及处理

Analysis and Treatment of Acute Brain Bulging During Craniotomy which is Caused by Hematoma in Non-operative Field

目的:探讨非手术区血肿所致开颅术中所致急性脑膨出的原因和处理方法,以提高疗效。方法:回顾性分析近4年来经手术探查和术后头颅CT证实的该类患者24例治疗经过,就其发生原因和处理方法展开讨论。结果:开颅手术术中急性脑膨出原因很多,但是术中脑出血为主要原因,应密切分析出血原因,外伤要分析受伤机制,分析各种导致出血的因素。特别是对非手术区血肿要高度重视,对提高手术质量,改善预后有非常重要的作用。结论:应重视非手术部位的血肿所致急性脑膨出的情况。根据术前CT片头部受伤的机制来分析其它部位出血的可能性,术中探查或术后头颅CT检查对患者预后有非常重要的作用。

Objective：The authors of this paper discussed the reasons of acute cerebral bulging during craniotomy which is caused by hematoma in non-operative filed.Methods：The authors retrospectively reviewed 24 such cases during the last 4 years.All cases were operated on and confirmed by CT scanning post-operatively.Their treatment process was analyzed,the causes and treatment strategy was discussed.Results：Craniotomy for acute encephalocele There are many reasons,but in the brain of the main reasons for bleeding,should be closely analyzed the reasons for bleeding,traumatic injuries to analyse the mechanism analysis of the factors leading to bleeding.Especially on non-surgical hematomas should attach great importance to enhance the quality of surgery and improve the prognosis of very important role.Conclusion：Brain bulging caused by hematoma in non-operative field should take into account.Through pre-operative CT picture and the patient＇s mechanism of injury,we can think of the possibility of non-operative field hematoma formation.Intra-operative exploration and post-operative CT scanning is very helpful to the patient＇s prognosis.
