颅咽管瘤超微结构及Caspase-3蛋白表达的意义

摘要 目的:探讨颅咽管瘤超微结构及Caspase-3蛋白的表达和意义。方法:(1)采用Tunel法检测颅咽管瘤及正常脑组织的细胞凋亡情况,并用透射电镜观测颅咽管瘤组织超微结构及细胞凋亡表现;(2)采用免疫组化SP法检测50例颅咽管瘤及10例正常对照脑组织中Caspase-3的表达水平,分析Caspase-3表达的临床意义。结果:(1)Tunel法观察结果示颅咽管瘤组织中典型的凋亡细胞极少,凋亡指数小于1%,其临床意义很小;(2)电镜下釉质上皮型、鳞形上皮型瘤细胞超微结构有差异,而细胞凋亡不典型;(3)Caspase-3在颅咽管瘤中的表达程度强于正常对照脑组织(P=0.001),且Caspase-3的表达和分布与患者年龄、性别、病程、肿瘤大小均无相关性。结论:(1)釉质上皮型颅咽管瘤中核糖体和桥粒数量与星形细胞瘤相似,但没有大量的细胞膜突起和伪足、胶质细丝等,其具有浸润性生长的性质,但不及星形细胞瘤典型,提示釉质上皮型是介于良恶性肿瘤之间的中间型肿瘤;(2)Caspase-3在颅咽管瘤中表达,而在正常脑组织中则不表达;(3)颅咽管瘤中Caspase-3的表达与患者年龄、性别、病程、肿瘤大小等临床特征无统计学意义。 Objective:To investigate the significance of ultrastructure and Caspase-3 expression in craniopharyngioma.Methods:(1)The cell apoptosis in craniopharyngioma and normal brain tissue was inspected by mean of TUNEL technique and electromicroscope;(2)The expression of were detected in 50 cases of craniopharyngiomas and 10 cases of normal brain tissues by immunohistochemical SP technique and the significance of Caspase-3 protein expression was analyzed.Results:(1)The cell apoptosis was few and the apoptocal index was less than 1% in craniopharyngioma through Tunel way;(2)Electromicroscope observation revealed also one typical apoptosis cell,and coupling body and desmosome were more abunant in the cell of squamous papillary craniopharyngioma compared with adamantine epithelial craniopharyngioma;(3)The density of positive expression of Caspase-3 in craniopharyngioma was more powerful than normal brain tissue(P=0.001),and the correlation was not found between expression of Caspase-3 and patient age,sex,course of disease and the volume of tumor.Conclusion:(1)The numbers of ribosome and desmosome in adamantine epithelial craniopharyngioma were similar to astrocytoma,but there wre not a lot of cell membrane protuberance and pseudopod GFs(Glial Filaments)in the former,so craniopharyngioma grows less aggressive than astrocytoma and it is a osculant tumor between benign and malignant tumor;(2)The Caspase-3 protein is expressed in craniopharyngioma;(3)There was no significant difference between the expression of Caspase-3 and patient age,sex,course of disease and the volume of tumor.
