血清清蛋白影响维持性血液透析老年患者并发症的发生

The Effect of Serum Albumin Level on Complications in Elder Hemodialysis Patients

目的探讨血清清蛋白浓度对维持性血液透析老年患者并发症的影响。方法选取2005年1月～2006年12月我院维持性血液透析病人47例(年龄>65岁),根据血清清蛋白浓度分为2组,A组病人血清清蛋白<35g/L,B组血清清蛋白≥35g/L,观察病人病程中有无贫血、严重感染、心血管并发症及病死率。结果A组病人贫血及感染发生率显著高于B组(P<0.01),心血管并发症发生率高于B组,但未达到显著性差异(P>0.05),病死率明显高于B组(P<0.05)。结论在维持性血液透析的老年患者中,低清蛋白血症增加贫血、感染及心血管并发症的发生率,并增加患者的病死率。

Objective To investigate the impact of serum albumin level on complications in elder patients on maintenance hemodialysis. Methods A total of 47 patients（ Age 〉 65Y） on maintenance hemodialysis at our hospital from Jan 2005 to Oct 2006 was selected. All subjects were divided into two groups according to serum albumin level：group A （ALB 〈 35g/L） ,group B （ALB ≥35g/L）. Anemia, severe infections, cardiovascular diseases and mortality were considered as main complications. Results The incidence of anemia and severe infections were significant higher in group A than in group B （P 〈 0.01 ） ; The incidence of cardiovascular diseases was higher in group A than in group B,but there was no significant differences（P 〉 0. 05） ;and mortality was higher in group A than in group B（P 〈 0. 05 ）. Conclusion In elder MHD patients, hypoalbuminemia could raise the incidence of anemia, severe infections, cardiovascular diseases and death.
