摘要
目的:探讨胰岛素生长因子(insulin-likegrowthfactor-1,IGF-1)在卵巢浆液性肿瘤中的表达及与卵巢浆液性腺癌生物学行为、预后的关系,以期为该肿瘤早期诊断、治疗及判断预后提供依据。方法:应用免疫组化ELIVISON二步法,检测IGF-1在37例卵巢浆液性腺癌、16例卵巢交界性浆液性肿瘤、12例良性卵巢浆液性肿瘤组织中的表达,并对37例卵巢浆液性腺癌患者进行随访。结果:①卵巢浆液性腺癌、交界性浆液性肿瘤、良性卵巢浆液性肿瘤组织中IGF-1的阳性表达率分别为64.86%(24/37)、43.75%(7/16)、8.33%(1/12),卵巢浆液性腺癌组织中IGF-1的表达高于卵巢交界性浆液性肿瘤和良性卵巢浆液性肿瘤,卵巢交界性浆液性肿瘤又高于良性卵巢浆液性肿瘤,差异均有统计学意义(P<0.05)。②卵巢浆液性腺癌组织中IGF-1蛋白表达与病理分级、淋巴结转移相关(P<0.01),而与临床分期无关(P>0.05)。③卵巢浆液性腺癌组织中IGF-1蛋白表达的阳性率在3年以上生存组中均显著低于3年内死亡组(P<0.01)。结论:①IGF-1蛋白在卵巢良性浆液性肿瘤、交界性浆液性肿瘤、卵巢浆液性腺癌组织中阳性表达率呈逐渐升高的趋势,其表达差异有统计学意义,提示IGF-1参与了肿瘤由良性向恶性转化、发展,有望成为卵巢癌早期的诊断辅助标记。②卵巢浆液性腺癌中IGF-1蛋白表达促进癌演进,并与癌细胞转移、患者预后密切相关。
Objective: To study the expression of HIF - let in ovarian serous tumor and to investigate the relationship between the expression and biological behavior, prognosis of ovarian serous adenocarcinoma, to provide evidence for early diagnosis, treatment and prognosis of the tumor. Methods: The expression of IGF - 1 in 37 cases of ovarian serous adenocarcinoma, 16 cases of ovarian borderline serous tumor and 12 cases of benign ovarian serous tumor were detected by ELIVISON to follow - up the patients of ovarian serous adenocarcinoma mentioned above. Results: (1)The expressions of IGF - 1 were observed in 8. 33%, 43.75% and 64. 86% of benign ovarian serous tumors, ovarian borderline serous tumors and ovarian serous adenocarcinomas respectively. Expression rates of IGF - 1 in ovarian borderline serous tumors and ovarian serous adenocarcinomas were significantly higher than those in benign ovarian serous tumors ( P 〈 0. 05) . (2)Statistical analysis revealed that the expression of IGF - 1 was related to histological grade and lymphatic metastasis (P 〈 0. 01 ), whereas there was no relationship with clinical stage (P 〉 0. 05) . (3)The positive rate of IGF - 1 in survival group was much lower than that of the group of patients who died in 3 years ( P 〈 0. 01 ) . Conclusion: (1)The expression of IGF - 1 has a tendency of increasing in benign ovarian serous tmnor, ovarian borderline serous tumor and ovarian serous adenocarcinoma. IGF - 1 participates in the procedure of benign tumor transformed to malignant, which is expected to be the diagnostic - aid marker in early diagnosis of ovarian carcinoma.(2)IGF - 1 can promote the cancer progression, meaningwhile, it is strongly correlated with differentiation, metastasis and prognosis.
