肝动脉和门静脉栓塞化疗联合射频消融治疗原发性肝癌

Transcatheter hepatic artery chemoembolization combined with percutaneous portal vein chemoembolization and percutaneous radiofrequency ablation in treatment of patients with unresectable hepatocelular carcinoma

目的探讨肝动脉栓塞化疗、门静脉栓塞化疗及射频消融治疗不能手术切除的原发性肝癌的临床价值。方法对不能手术切除的56例肝癌患者行肝动脉栓塞化疗联合经皮门静脉栓塞化疗及射频消融综合治疗,并于同期单纯行TACE治疗的病人60例作对照。结果两组病人的肝功能变化差异无显著性意义(均P>0.05);两组病人治疗后ATP转阴率差异有显著性意义(P<0.05);CR和PR有显著性差异(P<0.01);1年及2年生存率差异有显著性意义(P<0.05)。结论对不能手术切除的肝癌患者行肝动脉栓塞化疗联合经皮门静脉栓塞化疗及射频消融综合治疗能显著提高AFP转阴率、总有效率及生存率,在临床上有较好的应用价值。

Objective To explore the clinical value of transcatheter hepatic artery chemoembolization（TACE） combined with percutaneous portal vein chemoembolization（PVCE） and percutaneous radiofrequency ablation（PRFA） in treatment of patients with unresectable hepatocelular carcinoma. Methods 56 patients with unresectable hepatocelular carcinoma were treated with TACE/PVCE/PRFA,and other 60 patients treated with TACE alone for control. Result After the treatment,the hepatic function tests were not significant different between the two groups （P〉0.05）. But the AFP negativity,the CR and PR rates,and one- and two-year survival rates in combination group were greatly higher than in control. Conclusion The combination therapy is useful for the patients with unresectable hepatocelular carcinomas.
