EOX和XELOX方案治疗晚期胃癌的疗效与毒副作用比较

EOX and XELOX combination chemotherapy for advanced gastric cancer

目的探讨表阿霉素联合臭沙利铂、希罗达的化疗方案(EOX)与奥沙利铂联合希罗达方案(XELOX)治疗晚期胃癌的疗效和毒副作用比较。方法对我院2002年5月～2005年4月采用EOX化疗方案治疗的晚期胃癌进行回顾性分析,并与奥沙利铂联合希罗达化疗方案(XELOX)进行比较。EOX组30例,XELOX组35例。结果EOX方案组总有效率为36.7%,XELOX方案组总有效率34.3%,两组间无显著性差异(P= 0.84);XELOX方案组手足综合征的发病率较EOX方案组商,但大多都为Ⅰ～Ⅱ度反应。而Ⅲ～Ⅳ度白细胞减少、恶心呕吐、脱发的发病率较EOX低,两组差异有统计学意义(P<0.05)。结论XELOX和EOX方案治疗晚期胃癌的疗效相近,毒副作用可耐受,在一些年老体弱或多次化疗后骨髓耐受差的患者可能更适合XELOX方案。在临床方案选择上,可根据不同患者的实际情况进行选择。

Objective To compare the efficacy and toxicity of the regimen of EOX （epirubicin, oxaliplatin and xeloda） with XELOX （oxaliplatin, xeloda） in treatment of patients with advanced gastric cancer. Methods Data of patients with advanced gastric cancer treated with EOX （ n = 30） from May 2002 to April 2005 were retrospectively analysed and compared with those treated with XELOX （n = 35）. Results The overall response rate was 36. 7% in EOX group and 34. 3% in XELOX group （P 〉 0. 05 ）. The main complication was hand -foot syndrome （grade I - Ⅱ ） in XELOX group and leukopenia （grade Ⅲ- IV）, nausea, vomiting, and alopecie in EOX group （P 〈 0. 05）. Conclusion EOX regimen is as effedive as XELOX in the treatment of advanced gastric cancer with tolerable toxicities. However, XELOX regimen is better suitable for the patients who are elder and weaker or with leucepenia after chemotherapies.
