摘要
目的研究角质细胞生长因子及微血管密度在子宫内膜异位症患者卵巢异位组织、在位内膜中的变化及其表达与患者的临床分期及月经周期的关系。方法应用免疫组织化学链霉菌抗生物素蛋白-过氧化物酶连接法检测30例子宫内膜异位症卵巢巧克力囊肿患者的卵巢异位组织及在位内膜中角质细胞生长因子的表达,根据CD34在血管中的表达计算组织的微血管密度。结果角质细胞生长因子在正常内膜、子宫内膜异位症在位内膜及异位内膜组织中的表达阳性率逐渐升高(3组比较H=31.02,P<0.01)。正常内膜及子宫内膜异位症在位内膜中角质细胞生长因子表达受月经周期影响,分泌期的阳性表达率高于增生期(正常内膜中两组比较P=0.001,卵巢巧克力囊肿中两组比较P=0.017,均P<0.05)。在正常内膜、子宫内膜异位症在位内膜及子宫内膜异位症异位组织中的微血管密度逐渐增加(H=79.13,P<0.01);正常内膜及子宫内膜异位症在位内膜分泌期的微血管密度高于增生期(正常内膜中两组比较t=5.28,在子宫内膜异位症中角质细胞生长因子、微血管密度二者密切相关(r=0.73)。结论角质细胞生长因子的表达及微血管生成增加与子宫内膜异位症的发生有关,二者在子宫内膜异位症的发生发展中密切相关,可能为子宫内膜异位症的治疗提供新的靶点。
Objective To investigate expression of keratinocyte growth factor (KGF) and change in microvessel density (MVD) in eutopic and ectopic endometria of women with endometriosis (EMs) and their correlation with clinical staging and menstrual cycle of the women. Methods The expressions of KGF and CD4 were detected by using immunohistochemical SP methods. MVD was calculated according to expression of CD34 in the vessels. Results The positive expression rate of KGF in normal endometria, eutopic and ectopic endometia in endometriosis was increased orderly(H = 31.02, all P 〈 0.01 ). The expression of KFG both in normal endometria and eutopic endometria in endometriosis was affected by menstrual cycle of the women: that in secretory phase was 92.3% and 85.71% in proliferative phase respectively (in normal endometrium group: P =0.001 ; in chocolate cyst of ovary group: P =0. 017). The expression of KGF was not associated with clinical stage of endometriosis ( P 〉 0.05 ). Also, MVD in normal endometria, eutopic and ectopic endometia in endometriosis showed an orderly increased tendency (H = 79.13, all P 〈 0.01 ), and that in secretory phase of either normal endometria or eutopic endometfia of endometriosis was higher than that in proliferative phase (in normal endometrium group: t = 5.28; in eutopic endometria of endometriosis group : t = 6.42, both P 〈 O. O1 ). Although angiogenesis was seen in every phases of menstrual cycle, no associations between angiogenesis and clinical stages were found (P 〉 O. 05). In endometriosis, the expression of KGF and MVD were closely correlated with each other ( r = O. 73 ). Conclusion The expression of KGF and increased microvessel angiogenesis are associated with development of endometriosis and also the expression of KGF and MVD are closely correlated with each other in occurrence and development of endometriosis, which may provide new targets for treatment of endometriosis.
