Clinical Significance of Microvessel Density and Angiogenesis-related Factors Expression in Laryngeal Carcinoma

摘要 目的:探讨喉癌组织微血管密度(microvessel density,MVD)、血管内皮生长因子(vascu-lar endothelial growth facor,VEGF)、缺氧诱导因子-1α(Hypoxia inducible factor 1 alpha,HIF-1α)和P53蛋白的表达及临床意义。方法:通过免疫组化SP法对64例喉癌标本中VEGF、HIF-1α、P53蛋白和MVD进行了检测。结果:喉癌中VEGF、HIF-1α、P53蛋白的阳性表达率分别为71.88%(46/64)、65.63%(42/64)和62.50(40/64)%,MVD为35.79±9.49,P53蛋白与HIF-1α及VEGF之间呈正相关(r=0.323,P=0.009<0.01;r=0.305,P=0.014<0.05),HIF-1α和VEGF呈正相关(r=0.279,P=0.026<0.05),它们与颈淋巴结转移密切相关。而在有淋巴结转移的肿瘤中VEGF、HIF-1α及P53阳性表达率及MVD明显高于非转移组(P<0.05)。结论:VEGF、HIF-1α和P53蛋白表达可能共同参予喉癌的血管形成,且VEGF、HIF-1α的表达可能由P53蛋白调节,共同参与颈淋巴结转移的过程。 Objective: To evaluate the correlations of microvessel density ( MVD ) , vascular endothelial growth facor (VEGF), Hypoxia inducible factor 1 alpha (HIF- 1α) and P53 protein with clinical significance in laryngeal carcinoma. Method: Surgically resected specimens of 64 laryngeal carcinomas were studied by immunohistochemical stainning for VEGF , HIF - 1α, P53 protein and MVD. Result: Positive expressions of VEGF, HIF - 1α,P53 protein, were found in 71.88% ,65.63% , and 62.50% of laryngeal carcinoma specimens, respectively. The mean MVD in this series was 35.79 ±9.49. The expression of P53 and HIF- 1 was positively correlated with VEGF expression( (r =0.323 ,P =0.009 〈0.01 ;r =0.305 ,P = 0. 014 〈0.05) . The expression of HIF - 1 was positively correlated with VEGF expression( r =0.279 ,P = 0. 026 〈 0.05 ) ) . The expression of P53 , HIF - 1α , VEGF was significantly and positively correlated with lymph nod metastasis. The microvessel density (MVD) in P53 - or HIF - 1α and positive tumors was significantly higher in patients with cervical lymph node metastasis than those without metastasis ( P 〈 0.05 ) . Conclusion: The P53 protein , HIF - 1α, VEGF may play an important role in the angiogenesis of laryngeal carcinoma . Furthermore, VEGF and HIF - 1α expression may be regulated by P53, metastasis in the process of laryngeal cancer cell spreading to cervical lymph nodes.
