唇腭裂术后成人颌面部畸形的序列治疗:附4例分析(英文)

目的:通过研究因唇腭裂手术导致颌面部畸形的治疗,探讨序列治疗的方式及效果。方法:4例唇腭裂患者在幼儿时进行唇腭裂治疗,成人后出现颌面部畸形。通过术前正畸、正颌手术、牙槽突裂植骨、术后正畸治疗、义齿修复以及软组织矫正等6个阶段,对畸形进行序列治疗。通过面型分析、头影测量分析,探讨颌面部畸形的矫治效果。结果:所有患者的关系良好,关系稳定,未出现复发。面部容貌和软组织形态得到较大改善。结论:通过序列治疗,唇腭裂手术所致的成人颌面部畸形可得到良好的矫治。

PURPOSE： To study the effect of sequential procedure performed to correct the deformities resulting from cheiloplasty and palatoplasty. METHODS： Four adult patients with facial deformities resulting from cheiloplasty and palatoplasty during their childhood underwent the sequential procedures, including presurgical orthodontics, orthognathic surgery, bone grafting during surgery, postoperative orthodontic treatment, denture and prosthodontics, and correction of soft tissue. RESULTS： In all patients, the occlusal relationships and soft-tissue profiles were greatly improved. All patients underwent orthodontic treatment as soon as the surgical procedure was completed to achieve good occlusal relationships. After an average follow-up period of 30 months, the maxillary position and occlusal relationship of all patients were stable, and no obvious relapses were seen. CONCLUSIONS： Among the patients who received cheiloplasty and palatoplasty during their childhood, the abnormalities often become more and more serious while they are growing up. Sequential treatments after the growth spurt and adolescence are necessary. During the procedure, the coordination of surgical and orthodontic procedure is particularly important.
