远距离迁移人员肺部细菌感染的CT表现

CT Findings of Bacterial Pneumonia in People with History of Long Distance Migration

目的:探讨青壮年人不典型细菌性肺炎的CT征象,以提高对远距离迁移人员肺部感染的认识。方法:回顾性分析26例经临床确诊的远距离迁移人员肺部细菌性炎症的影像表现,重点分析其CT表现,所有患者均接受抗生素治疗而痊愈。结果:26例中单肺叶或肺段病变9例,其中6例病变密度不均匀,边界模糊;多发性病变12例,均累及2个或2个以上肺叶,病灶呈大片状、结节状或片条状密度增高影,3例病变局部呈磨玻璃样改变;局灶性病变5例,4例表现为卵石样或蜂窝状局限灶浸润,另1例则呈单纯球形高密度肿块。结论:远距离迁移青壮年人群肺部细菌感染的影像表现多不典型,诊断时需密切结合病史、临床表现、实验室检查及流行病学特征进行综合分析。

Objective：To study the CT findings of atypical bacterial pneumonia in young and middle-aged patients,in order to improve the understanding of lung infection in patients with history of long distance migration. Methods： The imaging findings of clinically diagnosed bacterial pneumonia in 26 patients with the history of long distance migration were retrospectively analyzed,mainly concentrated at CT findings. All of the patients were cured by antibiotic therapy. Results： Among the 26 patients,lesion located in one single lobe or segment of lung （n= 9）, with non-homogeneous and ill-defined lesions in 6 of these 9 patients. Multi-focal lesions were assessed in 12 patients,with two or more pulmonary lobes were involved. Most of the lesions showed massive,nodular or linear opacities,ground-glass opacities were revealed in 3 patients. Focal lesion was showed in 5 cases,with cobblestone or honeycomb-like infiltration （n=4） and high attenuated spherical mass opacity （n=1）. Conclusion：Most of the bacterial pneumonia in young and middle-aged patients with the history of long distance migration had atypical imaging manifestations. Accurate diagnosis might be obtained with comprehensive analysis by correlating with personal history,clinical and laboratory findings,epidemiology as well as imaging manifestations.
