β-cat及EGFR在胃癌癌变中的表达及生物学意义

摘要 目的:探讨β-连环素(β-cat)及表皮生长因子受体(EGFR)在胃癌癌变过程中的表达及生物学意义。方法:建立组织芯片(Tissue Microarry,TMA)技术平台,应用该技术和免疫组织化学SP法检测两种蛋白在胃癌石蜡包埋组织中的表达。结果:组织芯片制备成功。β-cat在四组中的阳性率分别为93.3%、57.7%、55.0%、49.2%,其中胃癌组、胃上皮非典型增生组、胃炎组与正常组比较差异均有统计学意义(P<0.01、P<0.05和P<0.05)。EGFR在四组中的阳性率分别为6.7%、19.2%、45.0%、56.9%,胃癌组与正常组和胃炎组比较差异具有统计学意义(P<0.01和P<0.05)。胃上皮非典型增生组与正常胃比较差异有统计学意义(P<0.05)。两种蛋白的表达与胃癌的组织分化程度和淋巴结转移显著相关,而与年龄、临床分期等无明显相关(P>0.05)。β-cat与EGFR二者则呈明显负相关(P>0.01)。结论:本研究中β-cat的表达下降与EGFR的表达升高提示二者与胃癌发生密切相关,与胃癌组织分化程度和淋巴结转移密切相关,提示二者表达失衡在胃癌发展中发挥重要作用。因此,联合检测β-cat和EGFR可作为预测胃癌发生和发展的肿瘤标志物。 Objective: To investigate the expression and biological signicificance ofβ- cat and EGFR proteins in the process of malignant transformation of gastric cancer. Methods:Tissue mieroarray (TMA) platform was established. The two proteins above - mentioned in the paraffin - embedded gastric tissues are detected by TMA and the immunohistochemistry streptavidin- peroxidase (SP) method. Results: Gastric tissue microarray was succeed. The positive rates of β-cat protein expression were 93.9% ,57.7% ,55.0% and 49.2% in four types. There was significant difference between gastric carcinomas and normal gastric tissues (P 〈 0.01). There was also significance between dysplasia and normal gastric tissues (P〈 0.05) and between gastritis and normal gastric tissues (P 〈 0.05). Expression of EGFR protein was detected in 6.7 %, 19.2 %, 45.0 % and 56.9 % of four types. There was significant difference between gastric carcinomas and normal gastric tissues (P 〈 0.01 ) and between gastric carcinomas and gastritis(P 〈 0.05). The expression of two proteins were detected obviously positive correlation with carcinomas differentiation and with lymph node metastasis , but there was no significance in other aspects (P 〉0.05). There was obviously correlation between the β- cat and EGFR (P 〈 0.01). Conclusions:We had established tissue microarray platform. Our results suggested that the expressions ofβ- cat and EGFR are correlated with the promotion of gastric carcinomas. They are all correlated with differentiation and lymph node metastasis in gastric carcinomas. So they probably will be new indices used in screening and diagnosis of gastric carcinomas.
