桥本氏甲状腺炎分期的探讨

Study of phase of Hashimotos thyroiditis

目的：探讨桥本氏甲状腺炎（ＨＴ）分期在临床诊治和判断预后方面的价值；方法：对５０例ＨＴ和３１例淋巴细胞显著浸润Ｇｒａｖｅｓ病（ＧＤ）作了临床病理对比分析、形态计量学研究，并对３０例ＨＴ作了３～８年随访观察；结果：根据对以上资料的分析结果，把ＨＴ分作３期，即：（１）淋巴细胞浸润期（或称早期）；（２）甲状腺滤泡萎缩期（或称中期）；（３）纤维化期（或称晚期）；结论：ＨＴ分期反映了ＨＴ不同期的病变程度，为临床医生制定合理的治疗方案提供了理论和形态学方面的依据。

Purpose To explore the value of the phase of Hashimotos thyroiditis (HT) in the clinical diagnosis, treatment and prognosis of the disease. Methods The clinicopathological aspects and morphometric parameters were analysed in 50 cases of HT and 31 cases of Graves disease (GD) with prominent lymphocytes infiltrated. 30 cases of HT were of 3 to 8 years follow up to see the changes of thyroid function. Results Based on the data analysed above, HT were divided into three phases: (1) the phase of lymphocyte infiltration (or early phase); (2) the phase of follicular atrophy (or middle phase); (3) the phase of fibrosis (or late phase). Conclusions The phase of HT reflect the varied lesions in the different periods of the development of HT and provide the theoretical and morphological facts for the clinic doctors to make a proper plan.
