CK20mRNA和hTERTmRNA表达与胃癌患者外周血微转移的研究

The Relationship between CK20 mRNA and hTERT mRNA Expression in Peripheral Blood of Patients with Gastric Carcinoma and Tumor Micrometastasis

目的:检测胃癌患者外周血中CK20 mRNA和hTERT mRNA的表达,探讨其能否作为判断外周血微转移及预后的分子生物学指标。方法:采用RT-PCR技术检测40例胃癌患者、19例胃良性病变患者和20例健康体检者外周血中CK20 mRNA和hTERT mRNA定性和半定量表达。结果:1)CK20 mRNA和hTERT mRNA在胃癌患者中阳性率分别为52.50%(21/40)和65.00%(26/40),相对系数分别为0.81±0.17和0.78±0.17;在胃良性病变患者阳性率均为5.26%(1/19),相对系数分别为0.12和0.11;在健康体检者中均为0。胃癌患者与胃良性病变患者、健康体检者相比,两项指标均有统计学差异(P<0.01)。2)CK20 mRNA、hTERT mRNA阳性率和相对系数与临床分期、淋巴结转移、远处转移均有关(P<0.05),阳性者生存率低于阴性者(P<0.05)。3)CK20 mRNA、hTERT mRNA与CEA、CA199表达呈正相关(P<0.05),其阳性率高于CA199(P<0.05)。结论:胃癌患者外周血中存在CK20 mRNA和hTERT mRNA表达,与临床分期及患者生存密切相关,提示两项指标能预测外周血微转移和判断预后,其诊断外周血微转移优于CA199。

Objective： To detect the expression of CK20 mRNA and hTERT mRNA in peripheral blood of gastric carcinoma patients and to discuss whether these levels can be used as molecular biological markers for predicting micrometastasis and evaluating prognosis. Metheds： Peripheral blood samples of 40 patients with gastric carcinoma, 19 patients with benign gastric lesions, and 20 healthy cases were assessed by RT-PCR for qualitative and semi-quantitative analysis of CK20 mRNA and hTERT mRNA. Results： （1） The positive rate of CK20 mRNA and hTERT mRNA in gastric carcinoma patients was 52.50% and 65.00% and the related coefficients were 0.81±0.17 and 0.78±0.17, respectively. The positive rate of CK20 mRNA and hTERT mRNA in patients with benign gastric lesions was 5.26% and the related coefficients were 0.12 and 0.11. The two markers were not found in samples from healthy people. Significant differences were found in the expression of CK20 mRNA and hTERT mRNA in peripheral blood among patients with benign gastric lesions, patients with gastric carcinoma, and healthy people （P〈0.01）. （2）The positive rates and related coefficients of CK20 mRNA and hTERT mRNA were significantly correlated with clinical stage, lymph node metastasis, and distant metastasis （P〈0.05）. The patients with expression of CK20 mRNA and hTERT- mRNA had lower survival rates compared with those without expression of CK20 mRNA and hTERT mRNA （P=〈0.05）. Conclusion： CK20 mRNA and hTERT mRNA were detected in peripheral blood of gastric carcinoma patients. The two markers can be used to predict micrometastasis and evaluate prognosis because they are closely correlated with clinical stage and survival.
