早期和近期开展LC手术临床效果对比分析

Reviews the clinical data of the earliest 200 cases with the latest 200 cases of laparoscopic cholecystectomy

目的探讨腹腔镜胆囊切除技术的发展以及迅速安全开展腹腔镜胆囊切除术的方法。方法回顾分析我院自1996年开展腹腔镜胆囊切除以来最初200例和最近200例的临床资料。结果初期200例中有182例成功实施LC手术,手术时间38～150min,18例中转开腹,12例出现并发症;后期200例中有192例成功实施LC手术,手术时间20～170min,8例中转开腹,2例出现并发症。结论腹腔镜技术不断发展,LC的适应证明显扩大,手术简化、手术时间缩短,中转开腹与手术并发症发生率明显下降。迅速掌握这一技术除了遵循学习曲线规律还需建立一系列专业技术准入制度。

Objective To discuss the development of the technique in laparoscopic cholecystectomy and the way to perform this operation rapidly and safely.Methods The clinical data of the earliest 200 cases and the latest 200 cases of LC were retrospectively analyzed.Results In the first group 182 cases of LC were successfully accomplished with the operation time of 38～150min,while other 18 cases were converted to open surgery and 12 cases had complication.In the second group the opera-tion time was 20～170min,while 8 cases were converted to open surgery and 2 cases had complication.Conclusion The tech-nique of Laparoscope progress continuously.In order to control the technique,we should not only abide by the learning curve,but also build a series of technique admittance rules.
