阻塞性睡眠呼吸暂停低通气综合征30例的手术治疗

Obstructive sleep apna-hypopnea syndrome in 30 cases of surgical treatment.

目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的手术治疗。方法30例患者经常规检查发现均存在以口咽部狭窄为主的上气道阻塞,行单纯气管切开术1例,气管切开插管麻醉2例,行腭咽部成形术(UPPP)28例,UPPP联合舌骨上悬吊术2例。结果近期27例有效,总有效率90%,远期26例有效,总有效率86.7%,并发症包括插管窒息、出血、进食反流等。结论UPPP对以口咽部狭窄为主的OSAHS是行之有效的手术方法,对伴有严重舌肥厚的患者应同时行舌骨上悬吊术,应高度重视围麻醉期和围手术期并发症。

Objective To investigate the surgical therapy for obstructive sleep apnea-hypepnea syndrome （OSAHS）. Methods 30 patients all were found to have upper airway obsauction maily induced by narrow oropharynx, 1 of them was treated with simple tracheotomy,2 with trachentomy intubation tmesthesia,28 with uvulopalatopharyngoplasty（ UPPP） .2 of them treated with the combination of UPPP and Suprahyoid suspension;Results 27 patients were recently effective, and the total efficiency was 90 %. 26 cases were longterm effective, and the total efficiency was 86.7 %, complications were including intubafion asphyxiation, hemorrhage, eating reflux, etc. Conclusion UPPP is a well-established method of operation for OSAHS mainly induced by narrow oropharynx, those patients with hypertrophy of the tongue should be treated with the combination operation of UPPP and suprahyoid suspension. The great importance should be attached on anesthesia and perioperative complications.
