疑难病例析评 第165例 发热-干咳-呼吸困难

病历摘要
患者女，40岁。因发热，咳嗽2个月，呼吸困难3周于2008年5月27日收入北京协和医院呼吸内科。患者入院前20d因受凉后发热，体温38～39℃，干咳，少量白黏痰。青霉素及头孢曲松治疗无效，体温升至39．6℃。开始胸闷、气短。症状进行性加重，渐至不能下床。既往史：近1年来患抑郁症，进食量仅为平常1／4，体重下降10kg。无吸烟史。体格检查：身高165cm，
