摘要 目的探讨多普勒超声对甲状腺良恶性结节的诊断价值。方法多普勒超声观察甲状腺癌组39例和甲状腺良性结节组41例的内部血流特征,血流丰富程度分为0~Ⅲ级,Ⅱ级和Ⅲ级的血流形态分为点线型、团状型、分支型和穿入型;并测量血流峰值流速和阻力指数。结果Ⅱ级或Ⅲ级血流在甲状腺癌组阻力指数高于甲状腺良性结节组,分支型或穿入型的血管在甲状腺癌组中的检出率高于甲状腺良性结节组,甲状腺癌组中的阻力指数高于甲状腺良性结节组,差异均有统计学意义(P〈0.05)。结论超声对甲状腺良恶性结节有较高的诊断价值。 Objective To explore the value of color Doppler ultrasonography in the differential diagnosis of thyroid carcinoma and benign nodule. Methods The interior blood features of 39 cases with thyroid carcinoma and 41 cases with benign nodules were observed by color Doppler ultrasonography, then RI and blood peak flow rate were measured. Results There was higher detection rate ( grade 2 or grade 3) in thyroid carcinoma than that in benign nodules. The presences of penetrating vessel or branching vessel suggested thyroid carcinomas. RI of thyroid carcinoma group was higher than that of thyroid benign nodales group, there was significant differences between the two groups ( P 〈 0.05). Conclusion Ultrasound has important clinical value in the differential diagnosis of thyroid carcinoma and benign nodule.
