Impact of spontaneously hypertensive diabetes mellitus on myocardial cell structure and cardiac function in rats

摘要 目的:观察高血压合并糖尿病对心肌细胞结构及心功能的影响。方法:实验动物随机分为4组:对照组、自发性高血压(SH)组、糖尿病(DM)组和自发性高血压合并糖尿病(SHDM)组,每组各6只。SD大鼠和SH大鼠腹腔分别注射链脲佐菌素(STZ),结合高热量饲料喂养,复制DM和SHDM模型。16周后进行有创心功能检测及心肌细胞病理学检查。结果:SHDM组大鼠心肌细胞排列紊乱,细胞间隙减少,部分细胞变性坏死,并且有较多损伤的线粒体。SH组、DM组和SHDM组左心室舒张末压(LVEDP)较对照组显著增高(P<0.01),DM组和SHDM组左心室压力上升和下降最大速率(+dp/dMt ax、-dp/dMt ax)与对照组和SH组比较显著降低(P<0.01),而且SHDM组+dp/dMt ax和-dp/dMt ax显著低于DM组(P<0.01)。结论:高血压合并糖尿病对心肌细胞结构与心脏功能有严重的损害,且远大于单一疾病。 Objective: To observe the impact of on myocardial cell structure and cardiac function. spontaneously hypertensive diabetes metlitus (SHDM) Methods: Twenty-four SD rats were equally divided into an SD (control) , a DM, an SH and an SHDM group. DM and SHDM were induced in the SD and SH rats by intraperitoneal injection of streptozotocin combined with high fat diet, followed by invasive detection of the cardiac function and pathological examination of the myocardial cell structure 16 weeks later. Results: Disorganized array, decreased interspaces, scattered degenerate necrosis and damaged mitochondria were observed in the cardiac cells of the SHDM group. The left ventricular end-diastolic pressure (LVEDP) was significantly higher in the SH, DM and SHDM groups than in the SD group (P 〈 0.01 ), while + dp/dtMaxand - dp/dtMax significantly lower in the DM and SHDM than in the SD and SH groups (P 〈 0.01 ), as well as in the SHDM than in the DM (P 〈 0.01 ). Conclusion : A combination of hypertension and diabetes melIitus does far more damage to the myocardial cell structure and cardiac function than either hypertension or diabetes mellitus alone.
