16例眶内容剜出的原因及病理学分析

The reason and pathological analysis of 16 patients with orbit evisceration.

目的分析眶内容剜出的病理学分类以进一步探讨眶内容剜出的原因。方法对2001年1月至2007年8月眼病理室存档的16例眶内容剜出病例进行临床分析及组织病理学分析。结果恶性肿瘤15例,占分析病例的93.75%,其中眼睑皮脂腺癌眶内蔓延6例(37.50%)、眼睑鳞状细胞癌眶内蔓延4例(25.00%)、视网膜母细胞瘤眶内转移3例(18.75%),黑色素瘤2例(12.50%);眶蜂窝织炎1例;占分析病例的6.25%。结论眶内恶性肿瘤是眶内容剜出的首要原因,最常见的是眼睑皮脂腺癌和鳞状细胞癌眶内蔓延,早期正确的诊治可避免眶内容剜出。

Objective To analyze the pathological classification and the cause of orbit evisceration. Methods 16 cases of orbit evisceration from January 2001 to August 2007 in Henan Eye Institute were reviewed retrospectively, the clinical data and pathological diagnosis were analyzed. Results 15 eyes（93.75% ） of 16 eyes with malignant tumor were found, ineluded 6 eyes（37.50% ） with orbital extension of palpebral sebaceous gland carcinoma, 4 eyes（25.00% ） with orbital extension of palpebral squamous carcinoma, 3 eyes （18.75%） with orbital metastasis of retinoblastoma, 2 eyes （12.50%）with orbital melanoma; orbital cellulitis was found in another eye （6.25 % ）. Conclusion The main cause of orbit evisceration is malignant tumor in orbit. Orbital extension of eyelid sebaceous gland carcinoma and squamous carcinoma are the frequent reasons. Early diagnosis and correct treatment can help us avoid orbit evisceration.
