胃肠道间质瘤临床病理诊断及鉴别诊断 被引量:2

摘要 目的准确认识胃肠道间质瘤(GIST),掌握胃肠道间质瘤的诊断及鉴别诊断。方法收集外科手术标本102例胃肠道间叶源性肿瘤,其中确诊82例GIST对其进行回顾性分析,重新切片、染色、分析观察,选择肿瘤细胞丰富的蜡块连续切片,每例均用CD117、CD34、SMA、S-100、desmin抗体进行免疫组化SP法染色。结果82例GIST占同期消化道间叶肿瘤的80.4%,其中发生于胃65例,小肠6例,结肠2例,网膜、肠系膜9例。镜下:梭形细胞为主型63例,上皮样细胞为主型8例,2种细胞混合型11例。免疫组化显示CD117及CD34的阳性率分别为92.7%和76.8%、SMA、S-100蛋白灶性弱阳性为13.4%和7.3%,desmin均为阴性表达。结论GIST是消化道最常见的间叶性肿瘤,结合发病部位,CD117、CD34高表达率是GIST确诊的依据,SMA、S-100蛋白,desmin的联合使用可协助鉴别诊断。
