摘要
目的探讨子宫内膜样腺癌及癌前病变组织中PTEN、CyclinE的蛋白表达及其与子宫内膜样腺癌临床病理特征的关系。方法采用免疫组化SP法检测正常子宫内膜、内膜增生、内膜不典型性增生和子宫内膜样腺癌组织中PTEN和CyclinE蛋白的表达。结果PTEN和CyclinE蛋白在内膜不典型性增生组中阳性表达率分别为50.00%、60.00%;在子宫内膜样腺癌组中阳性表达率分别为45.45%和65.50%。两组中PTEN阳性表达均低于正常子宫内膜和子宫内膜增生组(P<0.01),而CyclinE阳性表达均高于正常子宫内膜和子宫内膜增生组(P<0.01)。PTEN和Cy-clinE蛋白的表达在内膜不典型性增生和子宫内膜样腺癌组中的表达均呈负相关(r=-0.61、P<0.05,r=-0.49、P<0.01)。癌组中PTEN阳性表达的缺失与组织学分级、肌层浸润深度或伴有转移及临床分期有关(P<0.01或P<0.05)。CyclinE的阳性表达率与组织学分级有关(P<0.01)。癌组中肿瘤的复发与PTEN的表达缺失和CyclinE的过度表达均有关(P<0.01)。结论PTEN表达缺失和CyclinE的过度表达涉及了子宫内膜样腺癌的发生、发展过程并与肿瘤的复发密切相关,二者的联合检测可作为子宫内膜样腺癌的早期诊断、判断肿瘤生物学行为的参考指标。
Objective To investigate the PTEN and CyclinE protein expression in endometrioid ma and precancerous tissues and their relationships to clinical pathological features of endometrioid ad Methods An S-P immunohistochemis adenocareinotry technique was used to measure PTEN and CyclinE protein in normal endometrium, endometrial hyperplasia, atypical hyperplasia and endometrioid ad Results PTEN - and CyclinE- positive in atypical hyperplasia group was 50.00% and 60.00%, respectively; and PTEN- and CyclinE - positive in endometrioid adenocarcinoma group was 45.45 % and 65.50 %, respectively. Both positive PTEN in these two groups were lower significantly than those in normal endometrium and endometrial hyperplasia groups ( P 〈 0.01), and positive CyclinE was also higher than those in in normal endometrium and endometrial hyperplasia groups( P 〈 0.01 ). The expression of PTEN protein was inversely correlated with CyclinE protein expression in atypical hyperplasia and endometrioid adenocarcinoma groups ( r =-0.61, P 〈0.05, r =-0.49, P 〈0.01). In endometrioid adenocarcinoma, the expression loss of PTEN protein was positively associated with histological grade, muscles invasion or lymph node metastasis and clinical stage ( P 〈0.05 or P 〈0.01). The expression of CyclinE protein was positively correlated with histological stage ( P 〈0.01). The recurrence in endometrioid adenocarcinoma group was associated with PTEN expression loss and overexpression of CyclinE( P 〈 0.01). Conclusion The results suggest that the loss expression of PTEN and overexpression of CyclinE protein are closely related to the genesis, development and recurrence of endometrioid adenocarcinoma. Combination detection of PTEN and CyclinE protein might be useful marker on early diagnosis, predicting the biological behavior and prognosis of endometrioid adenocarcinoma.
