用Sonoclot分析仪测评体外循环后凝血功能的改变 被引量:6

摘要 目的用Sonoclot凝血和血小板功能分析仪观察心脏直视手术体外循环前、后凝血功能的改变。方法选择20例体外循环下心脏手术患者,麻醉前及首次鱼精蛋白中和肝素后10 min取静脉血用Sonoclot分析仪检测全血凝血和血小板功能,并进行激活全血凝固时间(ACT)测定。结果体外循环前后ACT差异无显著性(P=0.27),而使用玻璃珠作为促凝剂的检测试剂盒(gb-ACTKit)术后明显高于术前(P<0.01),凝血速率(CR)术后明显降低(P=0.01),血小板功能(PF)降低但无显著差异(P=0.50)。结论Sonoclot分析仪能够早期检测体外循环后凝血功能的改变,是一种较为敏感的床边检测方法。 OBJECTIVE To evaluate coagulation function after extracorporeal circulation(ECC) with Sonoclot analyzer in open heart surgery. METHODS 20 patients were studied with activated clotting time(ACT) assay and Sonoclot analyzer before and after ECC. RESULTS ACT and Sonoclot values were in normal level preoperatively. After ECC, gbACT was significantly increased ( P = 0. 009) compared to the value before anesthesia, clot rate (CR) was significantly lower ( P = 0.01 ) than that preoperatively, platelet function (PF) was slightly decreased ( P = 0.50), and ACT was slightly increased ( P = 0.27). CONCLUSION Sonoclot analyzer is able to diagnose early coagulation dysfunction after ECC. It's more sensitive than ACT.
