超声引导技术应用于锁骨上臂丛神经阻滞 被引量:9

摘要 目的:探讨超声引导下锁骨上臂丛神经阻滞的临床效果。方法:183例行择期上肢手术的病人,均在超声引导下行锁骨上臂丛神经阻滞,局麻药为0.75%罗哌卡因与2%利多卡因等量混合液0.4ml·kg-1。观察操作时间、起效时间及桡神经、腋神经、正中神经、尺神经、臂内侧皮神经、前臂内侧皮神经、臂外侧皮神经、锁骨上神经的阻滞效果;评定手术全程的麻醉效果及镇痛时间。结果:操作时间为(3.8±1.6)min,起效时间为(4.3±2.1)min,桡神经、腋神经、正中神经、尺神经、臂内侧皮神经、前臂内侧皮神经、臂外侧皮神经7根神经分支阻滞效果相同(P>0.05),锁骨上神经阻滞完善率低于以上7根神经(P<0.05)。麻醉优秀率为96.6%、良率达100%,镇痛时间为(450±110)min。结论:超声引导锁骨上臂丛神经阻滞操作时间短、阻滞效果完善,有临床推广应用的价值。
