数字化无创血液动力学监测系统在老年原发性高血压患者中的应用 被引量:3

摘要 目的:应用Bioz.Com数字化无创血液动力学监测系统,检测不同脉压水平的老年原发性高血压患者的血液动力学状态,为临床治疗提供帮助。方法:选取32例无高血压的老年人为健康对照组(A组);选取61例老年原发性高血压患者为高血压组(B组)。B组又依据脉压水平分2组:B1组(脉压<70mmHg,31例);B2组(脉压≥70mmHg,30例);应用Bioz.Com系统检测并比较不同组患者的血液动力学参数。结果:(1)B1组与A组相比,总动脉顺应性指数(TACI)明显下降,全身血管阻力指数(SVRI)明显升高(P均<0.05);(2)B2组与A组相比,除以上两项外,心脏指数(CI)、每搏量指数(SI)、心肌加速指数(ACI)明显下降,收缩时间比率(STR)明显升高(P均<0.05),左室射血时间(LVET)缩短亦较明显,但达到统计学标准;(3)B2组与B1组相比,TACI、ACI明显下降,SVRI、STR明显升高(P均<0.05)。结论:老年原发性高血压患者中,脉压明显升高者的血液动力学改变更加显著,更易引起心肌损伤。 Objective: To evaluate hemadynamic characteristics of essential hypertension patients with different pulse pressure by Bioz. Com noninvasive hemadynamic monitor system. Methods: All subjects were divided into three groups according to the blood pressure and pulse pressure. Hemadynamic parameters were measured and compared among lifferent groups. Results: (1) Compared with group A, patients in group B1 had a lower arterial compliance index (TACI) and a higher systemic vascular resistance index (SVRI) (P〈0.05 all) ; (2) Compared with group A, patients in group B2 had a lower TACI, cardiac index (CI), stroke index (SI), acceleration index (ACI) and a higher SVRI, systolic time rate (STR) (P〈0.05 all) ; (3) Compared with group B1, patients in group B2 had a lower TACI, ACI and a higher SVRI, STR (P〈0.05 all). Conclusion: Patients who had a higher pulse pressure often have more significant haemodynamic changes and cardiac muscular injury.
