基底细胞样型乳腺癌形态表现及其临床病理特点 被引量:2

摘要 目的观察基底细胞样型乳腺癌形态特点,探讨基底细胞/肌上皮标记物表达与临床病理的关系。方法对84例组织学分级为Ⅱ、Ⅲ级的乳腺癌进行免疫组化染色,并分为5组:A组(ER+/C-erbB-2-),B组(ER+/C-erbB-2+),C组(ER-/C-erbB-2+),D组[ER-/C-erbB-2-且表达至少一种基底细胞和(或)肌上皮标记物],E组(ER-/C-erbB-2-且不表达基底细胞和肌上皮标记物)。对比观察D组与其它4组形态学差异,同时观察A、B、C3组基底细胞标记阳性与阴性病例间形态上的差异。结果基底细胞样型乳腺癌形态学表现:癌细胞弥漫分布,可被纤维分隔,有多少不等的淋巴细胞浸润,中央常见大片瘢痕状无细胞区。细胞异型性明显,胞核大且多呈空泡样,有大核仁。结论基底细胞样型乳腺癌形态学表现异型性明显。 Objective To summarize the morphological features of based-like subtype of invasive breast carcinoma (BLSIBC), and study the relation between markers expressing and clinical pathology. Methods Immunohistochemistry was pefformed in 84 cases of breast carcinoma with grade Ⅱ and Ⅲ ,and five groups are classified: group A (ER+/C-erbB-2-) ,group B (ER+/C-erbB-2+),group C (ER-/C-erbB-2+), group D (ER-/C-erbB-2 and expressing at least one kind of basal cell or myoepithelium markers,group E (ER-/C-erbB-2- and not express- ing any kind of basal cell or myoepithelium markers). The differences in microscopic features are compared and summarized between group D and other four groups, and between the cases with CK5/6+ and CK5/6-. Results the morphological features of BLSIBC are summarized as followings: With marked pleomorphism, carcinoma cells have large karyons and their nucleoli are large and spumescent. Separated by fibrous septa, diffuse carcinoma ceils are surrounded by sparse tumor infiltrating lymphocytes, and large scarring areas without cells are seen. Conclusion The morphological features of BLSIBC possess marked pleomorphism, but basal cell or myoepithelium markers are independent of Malignant degrees in non-BLSIBC.
