语前聋儿童人工耳蜗植入的特点和效果比较研究

摘要 目的探讨语前聋儿童人工耳蜗植入的特点并比较植入不同机器后听功能的变化特点。方法以接受Nucleus N24和Clarion S人工耳蜗植入的47例语前聋儿童为研究对象,对比研究2组在手术操作中的不同点,并采用Nottingham大学听觉活用进步教育听力进程评价体系(LIP)探讨研究2组患儿在术后不同时期的听功能变化。结果Nucleus N24组手术损伤小,Clarion S组插入电极的比率高;2组在术前及开机3,6,12个月时的LIP平均得分Nucleus N24组为(8.5±9.9),(35±8.6),(39±2.4),(40±2.9)分,Clarion S组为(5.6±6.5),(28.1±13.4),(34.4±9.2),(39.8±3.5)分。开机后的最初3个月听功能显著提高,3~12个月时提高稍缓慢。结论开机1年后Nucleus N24组和Clarion S组的听功能都接近满分,语前聋儿童接受人工耳蜗植入后听功能可获得不同程度提高。 Objective To study the characteristics and changes of the auditory function in prelingual children with cochlear implantation in different periods. Methods 47 children with prelingual were studied after Nucleus N24 and Clarion S cochlear implantation. The differences between these two operations were compared and the auditory changes in different periods 'after surgical procedures were discussed and evaluated hy the Listening Progress Profile of Nottingham University (LIP). Results Less damage was caused in Nucleus N24 group than in Clarion S group,but it was easier to operate in Clarion S group. The average LIP scores before and 3 months,6 months and 12 months after the operation were (8.5±9.9),(35±8.6), (39±2.4) and (40±2.9)in N24 group,and (5.6±6.5),(28.1±13.4),(34.4±9.2) and (39.8±3.5)in Clarion S group. The improvement of auditory function was significant in the initial 3 months after switch but slower from the 3rd to 12th month. Conclusion The auditory function can be improved in different degrees after the Nucleus N24 and Clarion S cochlear implantation in the children with pre-verbal deafness.
