The association of plasma ICAM-1 level and the severity of sepsis in rats

摘要 目的:探讨血浆细胞间黏附分子-1(ICAM-1)水平与脓毒症严重程度间的关系,寻找判断脓毒症预后的可靠监测指标。方法:60只SPF级SD大鼠随机分为假手术组(Sham组)和盲肠结扎穿孔组(CLP组),CLP组进行盲肠结扎穿孔术。以术后0、3、6、12、24和48h为观察点,观察各组大鼠术后活动、进食、竖毛、腹泻、眼球凹陷、呼吸等情况;活杀取血测定血浆IL-6、ICAM-1水平,并观察腹腔肠管扩张、充血、腹水、病灶包裹、肺表面充血等情况。结果:CLP术后大鼠血浆IL-6、ICAM-1水平于术后3h开始即明显升高,分别于术后6、24h达到高峰;CLP术后6hIL-6、ICAM-1水平与脓毒症严重程度评分间相关系数为0.929、0.920(P﹤0.01),而术后48h相关系数分别为0.136、0.852(P=0.52,P﹤0.01)。结论:脓毒症大鼠血浆ICAM-1水平与脓毒症大鼠严重程度明显正相关。 Objective To investigate the association of plasma intercellular adhesion molecule-1 (ICAM-1 ) level and the severity of sepsis in rats and look for a reliable predictor for the prognosis of sepsis. Methods Sixty specific pathogen-free SD rats were randomized to sham operation group (Sham group) or cecal ligation and puncture (CLP) group. The activity, food taking, piloerection, diarrhea, enophthalmos, and respiration were observed postoperatively in rats at hours 0, 3, 6, 12, 24, and 48. After sacrifice, levels of plasma interleukin-6 (IL-6) and ICAM-1 were determined, and intestinal distention and hyperemia, ascites, omental adhesion to the cecum, and pulmonary hyperemia were observed. Results Levels of IL-6 and ICAM-1 in the CLP group increased significantly 3 hours after the procedure (P 〈 0.05) and then peaked at 6 h and 24 h, respectively. The correlation coefficient (r) for the relationship of IL-6 level and the score of sepsis severity was 0.929 and that for the relationship of ICAM-1 level and the score of sepsis severity was 0.920 within the first 6 hours (P 〈 0.01), but was 0.136 and 0.852 at 48 hours, respectively (P = 0.52 and P 〈 0.01 ). Conclusion Plasma ICAM-1 level is highly positively correlated with the severity of sepsis in rats.
