高分辨MR观察语言前感音神经性耳聋患者听皮质 被引量:1

High-resolution MR study of auditory cortex in prelingual sensorineural hearing loss

摘要 目的探讨语言前感音神经性耳聋患者听皮质的时间依赖性变化,评价正常听力者和语言前耳聋患者的半球优势。方法对9~12岁、19~22岁年龄段语言前耳聋患者各20例以及相应年龄组正常听力者各20名行梯度回波MRT1WI薄层采集,分别测量听皮质、颞上回、颞叶的灰质、白质容积以及总容积。结果9~12岁、19~22岁年龄组语言前耳聋患者与正常听力者双侧Heschl回、颞上回、颞叶的灰质、白质容积以及总容积差异无统计学意义(P>0.05)。9~12岁组左、右侧颞叶、颞上回灰、白质容积比亦差异无统计学意义(P>0.05),而左、右侧Heschl回灰、白质容积比差异有统计学意义(P<0.05)。19~22岁组左、右侧Heschl回、颞上回灰、白质容积比差异有统计学意义(P<0.05)。耳聋患者和正常听力者听觉中枢的左、右侧半球容积差异有统计学意义(P<0.05)。结论与正常听力者相比,耳聋患者Heschl回灰、白质容积比均显著增加;耳聋患者与右利正常听力者听觉中枢的左侧半球容积显著大于右侧半球。 Objective To study the time-dependent changes and mechanisms of auditory cortex in prelingual sensorineural hearing loss, and to reveal the laterality of brain between hearing subjects and hearing-loss patients. Methods Two groups (9--12, 19--22 years old) of hearing-loss patients and hearing subjects underwent thin-slice anatomical TlWI examination. Results There was no statistical difference between 9--12,19--22 years old and hearing about gray, white matter and total volume of bilateral HG, STG, and TL (P〉0.05), while no statistical difference was found between the two groups of ratio of gray to white matter about TL, whereas statistical difference of ratio of gray to white matter about HG was noticed between 9--12, 19--22 years old hearing-loss and hearing, and 19--22 years old hearing-loss and hearing about STG (P〈0.05). The left volume of HG, STG, TL was statistically larger than that of right in 9--12, 19--22 years old hearing-loss and hearing subjects (P〈0.05). Conclusion The ratio of gray to white matter about HG in hearing-loss is obviously lager than that of hearing patients. The left volume of HG, STG, TL is larger than that of right in hearing-loss and hearing subjects.
