摘要
目的探讨乙型肝炎病毒前C区A1896变异与患者外周血CD4+CD25+调节性T细胞水平的关系。方法应用聚合酶链反应扩增包括HBV前C区在内的DNA片段,经纯化、末端标记后测序,判读结果;应用流式细胞仪检测外周血CD4+CD25+调节性T细胞表达水平。结果68例慢性乙型肝炎(慢性乙肝)、36例肝硬化和18例肝癌患者血清A1896位碱基突变率分别为41.2%、69.4%和100.0%;A1896变异株感染者CD4+CD25+调节性T细胞水平明显低于非变异株感染者(P<0.01)。结论A1896基因变异与乙型肝炎病毒感染后肝病的慢性化及临床病情加重,CD4+CD25+调节性T细胞表达以及患者免疫状态的改变相关。
Objective To analyze the relationship of hepatitis B virus precore gene nt1896 mutation and the expression of regulatory T cells in the patients with hepatitis B.Methods Precore gene A1896 mutation was detected by sequence analysis,expression of regulatory T cells was detected by flow cytometry.Results A1896 mutation rates were 41.2% in 68 patients with chronic hepatitis,69.4% in those with cirrhosis,and 100% in those with liver cancer.The expression of CD4^+CD25^+ regulatory T cells was lower in patients with A1896 mutation than that in those without(P〈0.01).Conclusion HBV A1896 mutation is associated with deterioration of chronic hepatic changes related to hepatitis B virus infection,the expression of regulatory T cells and inmune state of the patients.
