高频振荡机械通气治疗新生儿气胸20例 被引量:7

摘要 目的:探讨高频振荡机械通气(HFOV)合并胸腔闭式引流治疗新生儿气胸的临床疗效。方法:20例出现气胸的新生儿采用高频振荡通气并胸腔闭式引流治疗,观察其治疗前后血气指标的变化及临床疗效。结果:治疗12h后PaCO2由(10.10±3.25)kPa下降至(5.80±1.82)kPa(P<0.05),PaO2由(5.50±1.12)kPa上升至(7.22±1.65)kPa(P<0.05),24h后PaO2为(9.32±2.05)kPa(P<0.01),并且经皮氧饱和度和氧合指数、平均气道压等均逐渐下降。经HFOV并闭式引流治疗,20例气胸患儿完全治愈15例,放弃治疗3例,2例双侧气胸患儿治疗无效。结论:HFOV对治疗新生儿气胸并呼吸衰竭十分有效,且安全性好,比常频机械通气有很大优越性。
