对上海医疗卫生机构实施糖尿病综合防治管理具有代表性的五种模式进行研究与分析，了解五种管理模式特有的精华元素，为社区卫生服务中心开展糖尿病项目管理研究提供参考，使设计的社区糖尿病综合防治管理模式更适合社区患者的需要，能够达到长期、连续地监测与管理社区糖尿病患者的健康状况，促进社区糖尿病患者认知率、治疗率的提高，使血糖得到控制。

To study the five management models of the integrated prevention and cure of diabetes mellitus （DM） in medical institutions in Shanghai. Understanding the essential characteristics of the five modes helps provide reference information for the studies on DM program management conducted by community centers for health services. An appropriate management model of integrated DM prevention and cure for communities will better meet the needs of DM patients, have a long-term and continuous surveillance and control of health conditions of DM patients, and promote cognition rate, cure rate, and control of blood glucose in DM patients of the communities.
